Fees

I am an in-network TRICARE and Premera provider. I also bill other payors such as AETNA.

Out-of-pocket/out-of-network:

Formal Comprehensive Mental Health Evaluation, 90 min. plus: $250.00

50-75 minutes Initial Assessment: $150.00

50-60 minutes Individual Therapy: $100.00

Payment is due at the time of service. I accept cash, personal checks and credit cards. I can also provide clients an itemized bill detailing the services they received in a format that they may submit to their insurance company to request reimbursement. I strongly recommend that new clients contact their insurance providers to determine their policy’s out-of-network provider benefits.

I am available to contract for professional services.

Cancellations/No Show Policy

I ask that if a scheduled appointment must be canceled or rescheduled, clients provide 24-hour notice. I will allow one No-Show appointment without charge. Thereafter, unless emergency or unforeseeable situations arise, clients will be charged a fee for No-Shows. The fee is $50.
